Frequently Asked Questions

For your first appointment, please plan to arrive at least 30 minutes prior to your scheduled appointment time to fill out a patient information packet. Bring a list of current medications, your insurance card, photo ID, and any pertinent x-rays, MRI, CT, lab tests, etc. If your health insurance requires a written referral from your primary care physician, please have that on hand as well.

An appointment is required in order to visit the doctor. However, a same day appointment is possible. If you have an urgent situation, please call our office prior to visiting so that we may be prepared for your arrival.

Please call and let us know at least 24 hours in advance. We make a sincere attempt to honor your appointment time, however, the nature of our specialty sometimes makes delays unavoidable. If this is the case, please bear with us and rest assured that you will receive our utmost priority and individual attention as soon as possible.

Many managed care insurance programs require a referral to be issued by the patient’s primary care physician prior to seeing a specialist. In these cases, please verify that your referral has been authorized and issued. We cannot see patients without an appropriate referral. Unfortunately, insurance companies that require a referral will not cover or reimburse for services obtained without the required referral. Because all insurance contracts are between the patient and the insurance company, you are ultimately responsible for payment.

We participate with most major health insurance and HMOs. As a courtesy, we will file other insurance. However, you are responsible for payment. If you are unsure about your health insurance, please call our office to confirm. We recommend that you become familiar with your health insurance policy, its coverage, limitations, payments, and regulations concerning referral and pre-authorizations.

Please give at least 24 hours notice for all prescription refills. NO prescription refills will be made after hours or on weekends.

Insured patients should be prepared to submit co-payment and deductibles at the time of visit. Self-paying patients requiring extended services may inquire about a payment plan.
